MEDICAL NOTARY HOSPITALIZATION PACKAGE

Greg delivers compassionate care in this time of need.  The documents needed for the Hospitalization Package include the Medical Power of Attorney and a USB thumb drive with the signed, notarized document so you can make copies or upload it to hospital systems such as MyChart.  If you have your attorney’s documentation, that can be notarized at Bedsize, and if you don’t have any documentation, Greg carries generalized Power of Attorney Forms that can get you through until you can find an attorney who can create a more thorough document that you can use and replace the generalized Power of Attorney.

Remember that a licensed notary CANNOT UNDER ANY CIRCUMSTANCES GIVE LEGAL ADVICE, OR FORCE YOU TO SIGN ANYTHING.  NEITHER CAN YOUR FAMILY FORCE YOU TO SIGN ANYTHING AT BEDSIDE.  Please make sure you completely understand the documents and what you are signing.  IF GREG DETERMINES THAT YOU ARE NOT COHERENT ENOUGH TO UNDERSTAND WHAT YOU ARE SIGNING, GREG CAN CHOOSE TO CANCEL THE APPOINTMENT.  IF GREG DOES THIS, NO REFUNDS ARE ISSUED.  RULONA Notary Law states that only a licensed attorney can give legal advice.  A fully licensed notary public cannot give any legal advice.  Also, PA Law requires that the signer be coherent and know what they are signing.  The case manager may request an official Ombudsman, if necessary, to ensure the patient knows what they are signing.  This protects the patient and hospital system from any fraud.

The cost of a Medical Notary Hospitalization Package, excluding clerical or travel fees and after-hours or emergency bedside service, is $95.00.  This includes the notarization, all witnesses, the scan of the document, and saving it to ONE (1) USB thumb drive.  If you have your own documentation, use the upload feature on the contact page so that Greg can review the document first to make sure it has the correct notarial wording.

TECHNOLOGY SUGGESTION

Because Greg scans the Medical POA and puts it on a thumb drive, transfer that file to your personal caregiver’s phone, along with scans of your medical card, Medicare card, and any other important hospital information!  This way, you will always have it with you when you need it, and with iOS you can AirDrop it to another iPhone or email it when needed, so you can carry fewer items and always have a backup.
